What is the NDIS and how can Continue Care help?
The National Disability Insurance Scheme (NDIS) provides funding to eligible Australians living with disability to access supports that help them live more independently and achieve their goals.
Continue Care works alongside NDIS participants, families and support coordinators to deliver supports in a way that is practical, respectful and aligned with each person’s plan.
If you are unsure what your NDIS plan includes, we are happy to talk it through and help you understand how your funding can be used.
Do you only support NDIS participants?
Yes. Continue Care is a disability-only service provider and supports people funded through the NDIS.
Do you provide nursing or clinical supports?
Yes. We work closely with support coordinators, families and carers, as well as directly with participants. Collaboration is important to us, and we value clear communication and shared understanding to ensure supports are consistent and well coordinated.
